Cascadia invites feedback from those we serve
Survey is open during spring 2026
As part of our efforts to both amplify voices of participants and improve services, Cascadia Health is inviting feedback from those we serve.
Our annual feedback survey is open now. The survey asks participants what they think about Cascadia services. It can be completed on a computer or smart phone and takes about 5-10 minutes to complete. Participants also have chance to win a gift card!
As a data-driven organization, the information shared with Cascadia helps improve service delivery, our healthcare environment, and the ways we uplift the voices of the individuals we serve.
The survey is available in English, Spanish, Russian, and Vietnamese. We are able to provide surveys in other languages on request.
