Housing Outreach Team

The Cascadia Housing Outreach Team operates and administers a variety of permanent supportive housing in Multnomah County to more than 225+ households including single adults, couples, and families with children, including individuals without insurance or without income.  These programs provide a rental subsidy and supportive services to participants who have experienced homelessness and have a mental health disability. Services are mobile and provided in the participants’  homes or in the community with the intent of meeting participants where they are. Housing Outreach Team services are varied and include housing placement, housing retention, and mental health services including outreach and engagement, assisting in housing search and application processes, moving in, obtaining furniture, tenancy skills, advocacy with landlords, counseling, case management, support in accessing primary care, and addressing health needs.
