PORTLAND — Cascadia Behavioral Healthcare’s Street Outreach Team, a program that serves hundreds of people each year in Portland who are experiencing homelessness received a Wells Fargo grant of $6,150.

The Street Outreach Team reaches out to people who are living and sleeping outdoors to provide essential services and care coordination when they are needed most. The team connects people to shelters and provides hotel vouchers, emergency response services and programs to get people into permanent housing solutions. The role this team plays in the community and the services they provide are especially important during the winter months.

Wells Fargo’s grant to the Street Outreach Team will support the dedicated staff and the work they do every day in our community; connecting people to the services they need to survive and working to transition people out of homelessness and into homes.

About Cascadia’s Street Outreach Team

The team provides street-level outreach and dedicated in-reach to local shelters to assist people who are experiencing homelessness and struggling with mental health issues into stable housing.   The team’s primary goal is rapid placement into permanent housing.  Core services include:  Mental health assessments, crisis care coordination, assistance navigating service systems, referrals into short term housing options, and help accessing mainstream benefits.
