Whether or not you have health insurance, you can still get the care you need.
Cascadia Health accepts most insurance types. This includes Oregon Health Plan (Medicaid), Medicare, and some private insurance. Whether or not you have insurance, you may qualify for discounts based on family size and income. These discounts are through our Self-Pay program.
Starting July 6, 2026 our self-pay discount costs are changing.
If you currently pay for care through Self-Pay, we will contact you directly about these changes and how they might impact you. Review updated costs by service based on each Self-Pay tier. (See below to determine your Self-Pay tier.)
Self-Pay
If you do not have health insurance, or if your health insurance does not cover certain services, Cascadia offers care on a “sliding fee” discount based on family size and income.
Follow the below steps to find your Self-Pay tier.
After you find your Self-Pay tier, scroll down to see how much you would pay for common services.
- FAMILY SIZE: Find the row that shows the number of people in your household. “Household” means anyone that you would include on your taxes (even if you don’t file taxes). This could be a spouse, child, or any dependent.
- INCOME: Based on the row that shows your family size, find your annual household income (based on percentage of the Federal Poverty Level). Whichever column your income lands in, that is your Self-Pay tier.
EXAMPLE: If you live with your spouse and have 2 kids (and claim everyone on your taxes), your family size is 4. If you and your spouse’s combined annual income is $40,000, then you would fall under the “Self-Pay 2” tier.

The above fees do not represent a comprehensive list of services available for self-pay and are intended to provide an estimated cost of treatment based on the most commonly provided services. Fees are subject to change.
Payment may be made cash, check, Visa, MasterCard, Discover and AMEX. You will be provided with a receipt for your records. Please retain this receipt. Cascadia encourages you to become familiar with our policy and resolve all payments questions prior to receiving services.

Help applying for OHP
If you have trouble applying for the Oregon Health Plan, you can make an appointment with a Cascadia OHP assister. If you are interested in scheduling an appointment, call the main phone number of the health center nearest to you.
Cascadia has a No Surprise Act protocol to help those who pay out-of-pocket for services. This is a good faith effort to ensure that clients/patients are made aware of costs for services. Please contact a Cascadia staff member for more details.
