Potential Breach of Client Information 

On November 9, 2023, a breach of client information was discovered. The breach affected 24 clients. Below is more information about the breach.

The Health Insurance Portability and Accountability Act (HIPAA) requires that Cascadia Health report potential breaches of client information. A breach was discovered November 9, 2023, and confirmed on January 10, 2024. This breach affected 24 clients, and Cascadia was able to notify eight (8) individuals.

What Happened

A staff member working in the community left a notebook in a client’s home. The notebook was recovered the next day. The client whose home the notebook was left in reports they did not look at the notebook. However, limited information may have been exposed, including:

  • Client names
  • Client phone numbers
  • Client addresses (for some people)
  • Client dates of birth (for some people)
  • Description of reason for calling the Mental Health Crisis line

We are very sorry that this occurred, and we take client/patient confidentiality very seriously. This breach was reported and investigated, and all individuals involved in the breach that could be notified were immediately notified by phone or mail. Cascadia will continue to closely monitor and report potential breaches of health information.
