Mult. Co. Videos Highlight Cascadia Health’s Street Outreach Team
We all want a place where we feel safe, a place we can call our own. Cascadia Health’s Street Outreach Team helps individuals and families in our community find that place–and keep it. Multnomah County highlighted Cascadia street outreach staff in a video series launched this week, showing how our dedicated staff meet people where they are and bring hope and dignity to folks across our community.
The first video features Adele Atwood with the Street Outreach Team. A major aspect of Adele’s role is meeting people where they are, from case management to building relationships and sharing information about housing and health services.

About Street Outreach
The Cascadia Street Outreach Team meets people where they are to provide life-saving supplies and health and housing services. The team’s primary goal is to help people experiencing homelessness and mental health challenges with rapid placement into permanent housing. The team supports people experiencing homelessness who are living outside, in vehicles, and in local shelters.
Services include:
- Mental health assessments
- Crisis care coordination
- Support with navigating service systems
- Referrals into short-term housing options
- Help with accessing benefits.
People placed into independent housing by the Cascadia Street Outreach Team are provided with up to a year of home-based support services.
